Adult ADHD Diagnostic Package

Service Description

What is this appointment? This is a comprehensive adult evaluation to determine if you may have Attention Deficit Hyperactivity Disorder (ADHD). This includes BOTH a general new patient psychiatric evaluation and an ADHD evaluation. This thorough assessment uses multiple methods to provide an accurate diagnosis, including clinical interview, standardized questionnaires, screeners, and FDA cleared computerized testing (QbCheck). Who is this for? This evaluation is for those 18 years or older who are experiencing: Difficulty focusing or staying on task Trouble organizing or completing projects Forgetfulness or frequently losing things Restlessness or feeling constantly "on the go" Impulsive decision-making Challenges at work, school, or in relationships that may be related to attention or concentration What will this evaluation include? This package includes several components Patient interview: we will discuss your current symptoms, childhood history, and how attention difficulties are affecting your life Collateral information: with permission, we will gather information from someone who knows you well (a family member or partner) to understanding how your symptoms appear in different settings. QB Check computerized testing: You will complete an in person FDA cleared computerized test that measures attention, impulsivity, and activity level. This 20 minute test provides data that supports this diagnostic process.
